Privacy Policy
This Privacy Policy explains how Dallas Lifestyle Management (“we,” “us,” or “our”), a chiropractic clinic in Dallas, Texas, collects and uses information when you visit drcurrington.com (the “Site”) or send us an appointment or contact request. By using the Site, you agree to the practices described here.
1. Your health records
This policy covers the Site only. Health information you share as a patient at the clinic, including your records, intake forms and insurance details, is protected under federal law (HIPAA) and handled as described in the clinic’s Notice of Privacy Practices, which you can request at the front desk.
Please don’t send symptoms, medical history or other health details through the Site’s forms. Share them with your provider at your visit.
2. Information we collect
- Request details you enter in an appointment or contact form: your name, phone number, email address, whether you are a new or returning patient, the day and time you prefer, the service you are interested in, and any note you add.
- Usage analytics collected automatically when you browse the Site: pages viewed, approximate location, device and browser type, and the referring page. This data is collected without cookies and is reviewed in aggregate.
- Calls and emails you send the clinic directly.
3. How we use it
- To reply to your request and schedule your visit.
- To send a confirmation that we received your request.
- To understand which pages and services visitors look for, so we can improve the Site.
- To comply with legal obligations and enforce our Terms & Conditions.
We do not sell your personal information or use it for advertising.
4. Phone calls and text messages
When you send a request with your phone number, the clinic may call or text you at that number about your request and your appointment. These messages are about your visit only.
- Rates. Message and data rates may apply, depending on your mobile carrier and plan.
- Opt out. Reply STOP to any text to stop messages to that number. Reply HELP for help, or call (214) 520-1006.
- Your number stays with us. Phone numbers and text-message consent are never shared with or sold to third parties for their marketing.
5. Service providers we use
A few vendors help us run the Site. They may use the information only to provide their service to us:
- Protocol, the company that built and runs the Site, stores appointment and contact requests so the clinic can reply, and measures visits with cookieless analytics.
- Resend delivers the emails the Site sends, such as your request confirmation.
- Cloudflare hosts the Site and may cache pages at locations near you.
- Google Maps shows the clinic’s location on the Site. Google handles data from the map under its own privacy policy.
6. How long we keep it
Request details are kept as long as needed to reply and schedule your care, and for reasonable business records afterward. Aggregated analytics data is kept on a rolling 24-month window.
7. Cookies
The Site sets no cookies of its own. See our Cookie Policy for the small amount of information your browser stores while you use it.
8. Your choices
- Access, correction or deletion. You may ask what Site information we hold about you, and ask us to correct or delete it. We will respond within a reasonable time.
- Text messages. Reply STOP to any text from the clinic to stop them.
- California residents. The California Consumer Privacy Act gives you the right to know, correct and delete personal information we hold, and the right to equal service when you use these rights. We do not sell or share personal information for advertising. Email us with “California Privacy Request” in the subject.
9. Children
The Site is meant for adults. A parent or guardian should send requests for a child’s care. We do not knowingly collect personal information from children under 13.
10. Security
We use reasonable safeguards to protect the information you send through the Site. No internet transmission or storage system is fully secure, so we cannot guarantee absolute security.
11. Changes to this policy
We may update this Privacy Policy from time to time. The “Last updated” date at the top of the page shows the most recent revision.
